Herpes info please

I may have had sex with someone who has Herpes, but I don't know what to expect. I didn't see any scores on his genitals. Can someone who has this please let me know how easy it is to transfer, and how it has effected them or can effect them. I just really have no idea what that std is about, all I know is you can get them all over your lips and in your mouth.

Thanks for the info. I understand it can be ordered but I cannot
afford it on my own. I'd much rather pay a deductable and have it
prescribed.
I also understand that some people develop some side effects, but
i've also heard it's very succesful in others. I can only think of
trying it out or seeing what other topical solutions are available.

well ppl,,,,,,my man suprised me and showed up early! All i could
think was "oh crap, im not ready to tell him!!!!". Well, i did,
and,,,,,he took it great! told him everything i'de learned bout it
so far, answered his questions the best i could, and he said it was
ok! not sure what i was expecting, but figured i should prepare for
him running like hell! he laughed when i told him that, took me in
his arms, then told me i was to important to him for him to do
that : ). did find out that thers a good chance i got it from him
though. he did freak a lil when i told him abnormal paps are usually
from hpv. guess his ex gf had a few abormal paps yrs back! dont
know if her doc was a quack and didnt test her, did test her and it
was neg. or if she was positive and just didnt tell him!
Well, thats it for now. i get my coloscopy in a couple weeks, so
send all the positive thoughts my way that you can!

you can order it on the internet, you don't need to get it prescribed to you, the only thing is that it is very costly, and there are many side effects. I will not use it again it is very itchy and irritating

where to get aldara?

I have mentioned this to both my regular doctor and planned
parenthood yet neither have heard of it. What is the best method to
get this prescribed? Call up a dermatologist and ask them before
hand if they have heard of this? I'd really like to try it as
opposed to using other treatment types yet i don't want to have to
keep visiting doctors to find out whether or not they've heard of
this as i feel i'm running around in circles. Any recommendations?

You know, I found out that I had this about a week ago. I've always
been the type to use protection and not sleep around (I *rarely*
just 'hooked up' with girls - i've always been the caring type
looking for a relationship).
It's been a pretty rough week. I think it's more psychological than
anything because it really does make you feel like who the hell will
want to date me now?
The difficult thing is I know we're told this is a potentially 'good'
thing in that it separates the wolves from the sheep. Unfortunately,
that's not entirely true. I mean, most of us prior to this would
probably choose to not sleep with someone that has it - I don't think
that makes us a worse person than anyone else.
With that said though, it's comforting to hear of people that have
some understanding significant others. I only wish I was involved in
a relationship when i found this out, as opposed to being single in
an already depressed state with other shit going on in my life.
I am hopeful i'll get over this - it's still so difficult to think
that it's something I have. Every picture of it that i see on the net
is way more dramatic and visible than what i have.
Oh well, i figure when the time comes that i'm seeing someone, i'll
one day find someone that understands this. It's good to know there's
others out there like me though. It's crazy to think how many damn
people have this and it's just not known. It almost seems 'common' to
have it, scarily enough.
Welp, not much more to say in this unordered random message. Thanks

My doctor told me that instead of going to him to get the liq. N2 (freezing)
done, i can just go and get sum dry ice and do it myself. He sed to hold it
there until you see the skin go white about a mm or two around the wart. Just
thought i would let you guys know.

Unfortunately, warts do not have to be present to spread the virus. Although warts have a much higher content of the virus and are extremely contagious during an outbreak, that isn't the only time that you can pass it on. You body usually develops warts when you are "shedding" the virus, but not all the time. Therefor you can shed the virus without ever knowing it is happening. It could be at a time that you have chosen to have sex, even with a condom, and you still run a very high risk of infecting your partner. So he was mostly right ... but just kinda left some stuff out. Hope that helps a bit.

Today I went into see my gyno, his a very good german doctor
I was told about a week ago I had HPV and asked him about having
sex, he told me if the warts where not there and everything is clear
downstairs then I could have sex using a comdom but as soon as the
warts come back then I could pass it even using a comdon, he also
told me to take vitamins to boost my system, would love to hear from
anyone who has been told the same or maybe there is more I could
do to help my well being

The bottom line is that HPV is an extremely common viral infection. Many people will acquire the infection when they become sexually active. Most people never know that they have it and most will clear it or suppress it without its causing any problems. People who do have genital warts/condyloma can be treated at Campus Health. Some people, only women in this case, will have HPV infection on their cervix that causes a cellular response in the cervical cells called dysplasia. Usually the dysplasia is mild and gets better on its own in 12-24 months. If the dysplasia is more than mild it can be treated usually right in the Women's Health Clinic. We do know that many more people have HPV than ever get dysplasia. Smoking is a known co-factor that can help turn an HPV infection into dysplasia.

HPV should never be a life threatening infection. It should never turn into something more serious, like cervical cancer, with careful medical follow up.

my ex was diagnosed 5 months ago....we were together for 2 years and
never cheated on one another....that means at this point that ive had
HPV for at least 2 yrs and 7 months and if i didnt get it from
her...ive had it for 3 years....ive never had any symptoms....im
trying to move forward in my life....and its difficult in dealing with
this and especially in talking to potential partners.....
im wondering when i talk to them....in being honest with all
parties,,,,what is the possibility at this point of me passing HPV
along to any partner im with...

I have a bit of advice. First I would find a new gyne (get copies of your records from your gyne and take them with you to your new one). If I wasn't called about it, I would be really pissed (what's to say you can't trust them again). Our health is way to important to be shuffled around like that. We are always told that if we don't hear from them that we should presume everything is normal and we beleive that we can trust them. It's up to you but that's what I would do. Good luck. Angie

Well, welcome to the group (sorry that you have to be here, but glad to welcome you here as well). I am a newly diagnosed gal myself. I was dx'd in Sept. My pap came back abnormal so they did the colposcopy to confirm the pap results. What the colposcopy does is fairly simple (though it can be uncomfortable and hurt a bit). The dr. will have you in the usual position as you do for pap's. Then they will use the speculum to visualize everything, they spray a vinegar type solution (which burns like hell, just to warn you), this enable's them to see highlighted areas of suspicious lesions and they take a snip of those (which pinch, and if you are like me and my wonderful screwed up tilted cervix, then it may be a bit more difficult because mine kept slipping and was she was having a hard time snipping!) From what I have read, if the dr. see's any warts, then they may freeze them at that time. (I fortunately don't have any warts or didn't at the time of the biopsy). Hope

I think this might be more of a moral issue. Different people will
tell you different things -- that your body may have gotten rid of
it, or that you may still have it its just dormant and less
contagious, but really no one knows for sure. In fact theres been
instances where women go symptom free for years, and then it shows
up years later maybe when they are pregnant or something. And then
what, you would have to explain to your current partner at that
time. So it really depends on your outlook...personally i feel as
if the person I got it from didnt know he had it, and he swore that
every girl he slept with before that was a virgin, but what if one
them lied to him? Now i have to deal with this...I know its a hard
decision but this may be one case wehre honesty is the best policy.

Should I have an HPV test?

I am 30 or older -- Should I get the HPV test in addition to my Pap test?
In women 30 and over, screening using both an HPV test and a Pap test is more likely to find abnormal cervical cell changes than either test alone. If both tests are negative (normal), a woman may safely have her next Pap and HPV test in three years depending on her past Pap test findings and other risk factors. For this reason, some women now may have an HPV test when they have their Pap test. It will still be important to continue having routine recommended preventative health exams.

I am under age 30 -- Should I get the HPV test in addition to my Pap test?
No. HPV is very common in women under the age of 30 and cervical cancer is very rare in this age group. Most women under 30 with HPV will get rid of the virus without treatment. So including an HPV test along with your Pap isnt helpful for younger women and might be harmful if it resulted in too many tests and unnecessary treatment.

After age 30, HPV is much less common. If you are over the age of 30 and you test positive for HPV it is more likely that you may have gotten it many years before and your immune system hasnt gotten rid of it. Because HPV must be present for many years to cause cell changes, testing for HPV after the age of 30 is much more helpful.

I am under age 30, but my doctor suggested HPV testing after my Pap came back as "ASC-US." Why?
The most common abnormal Pap result is called ASC-US, or atypical squamous cells. About half of the women with ASC-US have these cell changes because of the presence of HPV, while the other half do not. If you have a follow-up HPV test after an ASC-US Pap result and the HPV test is negative, you probably do not need additional follow-up other than to repeat your Pap in one year. Usually only women with ASC-US testing positive for HPV need further evaluation. HPV testing is helpful at any age for determining which women with ASC-US need follow-up. This is different from using the HPV test with the Pap as part of your normal health visit.

Whoa, I've been lurking for some time, but damn! I think I'll be
moseying to this new board because yall are really damn depressing. I
was basically called a slut and told I somehow deserved this by the
doc that diagnosed me. Yeah, she couldn't find my supposed outbreak,
but she knew alright, it's cuz I'm a dancer, and we're all bad. If
anything, it's from my ex I was with for 2.5 years, thinking I was in
a monogamous relationship. Riiiiight....
Seriously though, it kicked my damn ass! However if I sat around
wallowing in everything going wrong in my life, I wouldn't get out of
bed. Has this altered my reality? Of course. However, I started doing
some reading, realized that my very own super kick ass body could
probably lick this. I kinda dig that, especially that it's working.
So now, once my body takes good care of it, and a *new* doc tells me
the coast is clear, I'm gonna go on with my life as normal, with one
small exception. I'm finally going to sit down with my partners, and
have the sort of "talk" that could have potentially prevented this
whole mess. Sucks it took a kick in the face to start that, but
whatever.

i admire your predisposition to successfully manage the 'problem' but many
of us are not in 'your' space. for many of us the wieght of coping with
this problem is overwhelming to say the least. i for one remain consumed
with the thought of being infected and never finding a mate or partner.
many of us just are not that secure with being with ourselves and being
alone. it's nice that one of us has the strength and conviction to manage
the problem. i for one have great difficultly reconciling myself to this
problem and generally feel unworthy and unwanted. i commend you on your
outlook however, i have to travel thousands of miles to get to your
destination. good for you . . .

You or your husband could have picked up the virus years ago. Many times after about three to six months after contracting the virus, signs begin so show. But it is very possible and most likely that one of you two picked it up before you were together and the virus simply remained dormant (sort of sleeping) in your body until recently. Or maybe, not trying to put bad idea's in your head, your husband brought it home from out of town. You know him fairly well I'd say since you've been together for six years ... so that is your call. Yes there are different kinds of hpv. They are generally classified in two groups. High risk HPV and Low risk hpv. High risk is the main cause for cervical cancer, and low risk causes warts. It is possible to be infected with multiple strains at a time though. What I am concerned about is how come your gyno is just now telling you that at the begining of the year your pap tested positive for hpv. That sounds very strange to me! In most cases
you will be contacted immediately if you test possitive for hpv b/c most cervical cancer is caused by this virus. If you do have it, living with it isn't so bad. Especially if you don't have the kind that causes warts. As long as you stay in good health and visit your gyno faithfully, it really isn't a big deal ... especially since you are already married. Herbal remedies work over long periods of time because they help give your immune system a boost. You don't want to be taking echenacia (spelling?) on a regular basis though b/c your body sort of builds up a tolerance too it, just like anything else one might take for a long time. So they help to boost your immune system, which is good to keep the virus in check. But it's not really gonna get rid of your warts or anything. You can use meds or let it go away on it's own. But the healthier you are, the better and easier it is to deal with.

It does the same thing in men that it does in women, only less often.
Men can get warts and penile or anal dysplasia or cancer.

Hi Group...

I was just wondering if anyone has had any of the same problems as I
have had. I have had hpv for over a year, with 2 different outbreaks.
After and during the seccond. I started to regularly have a rash in
my pubic area that what extremly itchy, red, and irritated. And would
sometimes lead to cuts. (which I was afraid would triger another
outbreak) now I went and was given medication to stop the irritation
3 times which helped but it would come back weeks later. Now I just
found a great gyno. who think but isn't sure about me having this
rear disorder that is common or caused by hpv. This is very hard to
treat and wont hurt me to much except for the rash simptoms which the
itchy and dryness are kinda like a yeast infection but much more
painfull. Has anybody ever has this, or simptoms like this. And for
how long, and was it just around your treatment time?
Any input would be great.

I had the same negative side effects as you have with Aldara cream. It really sucked because you spend all this money on the cream (unless you have insurance) and it doesn't work. All I can say is that I tried it the first time and I had 1 or 2 warts and it worked fine. About a year later I tried it again I had a larger outbreak and it did nothing at all but give me side effects. So I went to the liquid nitrogen treatment and it seemed to work the best for me. There are no sideffects. Also you can ask your doctor about this treatment. I can't think of the name but it is all natural made out of chinnese trees or somethings. It has a small side effect, of like a cut or scab that can be alittle painfull for a few days but it works really well you may only need 1 or 2 of the treatments.

Yes, anytime you "tear" the skin you run the risk of spreading the virus. I would think that waxing, although not likely to cut you, would still tear the skin.

I use "clippers" to cut really close. You may also want to consider laser hair removal in that area....

Well....here it goes.

I am a 38 year old male. I have been in a relationship for 16 years now. My partner and I were not exactly monogamous (we had primiscious sex). He talked me into trying 3 ways and even 4 ways. I was not comfortable with this at all. But I figured it's better than cheating on each other and I truly loved him. (I was so young and naive !!!)

This continued for a few years, about once every two to six months. Until I finally put my foot down. I am not a primiscious person by nature. (Truly)

I finally decieded after all these years that I wanted a child. I had tossed the idea around since my early 20's but he said he didn't want any children. So I tried to suppress my yearning for a child. Thats the background.

Now onto the here and now............

I finally made the decision that I wanted a child in my life, even if it meant giving up my relationship and raising a child on my own. I feel this all the way down to the core of my soul. However, About the middle of this year, my partner found a small pimple like bump on my penis and said that I should go to the doctor and find out what it is. I did and the doctor told me it was nothing. I even asked him if it was a venereal wart and he said NO. He put some solution on it and it went away. Then about two moths later, it reoccured. I went back and showed my doctor. His response was...."That's what happens with papilloma". I said..WHAT ? I then started doing some research. I have since found out that I can not have any children without infecting someone else. (My doctor didn't even have that information .....and still doesn't!!) My partner finally conceded to adpoting a child so that I could fufill my dream of having children.

Now, I find out that I have, not only genitial hpv, but ORAL hpv. (Soon to be announced by my doctor) Now I can't even kiss someone without the fear of infecting them as well.

To top this whole story off. (And I know this will sound strange) We have dogs and it appears that two of my dogs now have vaginal warts (one on each dog). They are small dogs and jump up in your lap and give kisses. (Thus sticking there toungue in your face and accidently getting your mouth) Now, this has made me not want to even adpot children because of the fear of infecting them as well. I would be too affraid to even give my child a kiss goodnight and then infecting them.

This whole situation has destroyed me. I have been questioning everything without any answers. I have been trying to gain as much knowlegde as I can, but it just isn't out there. Besides the obvious questions about hpv, I have been asking myself a lot of questions about my life in general.

Should I leave my partner or stay with him ? What if I can't find anyone else. What if I do and I infect them ? Can the hpv virus be passed on to animals ? Does this all mean that I can not touch, rub, kiss, caress, or even have sexual relations.....of any kind.....with anyone ? I am affraid to even let someone else touch me or kiss them on the cheek. I feel like I will have to live without the caressing of another person for the rest of my life.

I know this is all dramatic. I am so very sorry. I really am trying to stay postive. I just don't have any of my answers. My whole personality has changed since I first found out about this. My entire family, friends, and coworkers have been asking questions now for several months. I can't tell them. I used to be the go too person for everyone. The strong one that had all the answers or would at least give guidance and support. I feel weak and affraid. No one has any of the answers. That's why I have been looking for a support group within Southern California that I could go and talk too. I want to be happy and full of life again. I want to make everyone laugh again.

I don't even know how long I have had this or who I have exposed.

Well, thats my story. I hope that someone will have encouragement for me. I can't see amy sunshine and don't think I will for quite a while.

Erica, I know (we all know) how you feel... and you aren't alone... you have all of us here. It may take some time but you will accept it and move on (from the obsession). I ate, slept and breathed info on hpv when I first found out... the worry is still there but it's not my #1 worry (I have a 6 yr. old that I always have to worry about). There's no worrying til my next pap in March so I just take it day by day. It's still all so new to you and there will be a ton of questions that hopefully we can help you will or try to understand a bit. I do have to say that there are alot of unanswered questions out there, unfortunately.

information when they first find out about this, I speculate it has
to do with the fact that the majority of our doctors have nothing to
say that is worth listening to concerning hpv. Actually alot of
people in this group know more about it than their doctors do ... go
figure. But your on the right track ... knowledge is power! The more
you know, the easier it becomes. It's definately not the end of the
world or anything. Are you two on good terms? U and the guy you got

Sexual Ed only talked about chylamdia, gonarehea (sorry about the
spelling), syphillis, herpes, gential warts, AIDS and HIV. I didn't
know that HPV was the medical for this disease. I was under the
impression that genital warts was just that genital warts. We
weren't taught about there being 70+ strains or anything like that.
The teachers were afraid that they would offend someone as oppose to
educating us. I even took Human Sexuality in college and none of
these ever came up. My mother never really talked about it
except "DOn't Do It".

I have been treated and I only asked because what you said about it going away after 2 years was proven wrong in my own case..Ten years maybe nothing for the type or strand I am battling.

I guess I'd suggest figuring out if this is someone you want to be with for a while. If the answer is yes, then it shouldn't matter where or from whom you got HPV. I would just get treated and life a normal life. Good luck

I am sorry,the guys wife had it ten years ago he siad,and I was the only one he was with after...he could be lieing. but he was the only one who knew what it was..he is kinda not as honest as he could be...as far as being anyone else in my past sure thats possiable acording to everything I have read...you siad:

-All the research I've read and discussed with ASHA says that after 6 months to a year of being symptomless it is difficult to pass on the virus because being "symptomless" means that your body's immune system has eradicated the virus or decreased it to such low levels that it won't transmit.
For those of us who have the GW symptoms - you need to have lots of the "active" virus present to pass on the virus - meaning that you need to be in "formation of a wart" or
already have a wart-

What I am wanting to know is the extent to which the former boyfriend is lieing,is this a fact since I have read elsewhere and told by my doc that I will most likely always carry this..
I have it on the cells of my cervic ...no warts..just on the cellaur level.the guy I think I got it from's wife had it the same place same way..I don't know if that makes high risk or not.

Welcome to your new family! Everyone here is informative, supportive and all around great. I found out a couple months ago. I am just glad to meet another person who admits to being a hypochondriac (lol). I know I had a real hard time at first (and do when I let it get to me) I think about it alot but not to the point to where I am obsessing (in the beginning I researched so much on the net that I thought my cpu was going to short curcuit (lol) and I bought every book; I highly recommend 'what your dr. doesn't tell you about hpv and abnormal pap'. (if you want to know the author and the correct title, let me know). I asked everyone I knew that had abnormal pap's and found out some stat's that were overwhelming (however the outcomes have been successful for them all :)

to get cancer and die..and never be able to have children or get
married or "grow up". ive been, like i said, OBSESSING. i also have
convinced myself that te cramps im having are from the cervical
cancer. yeah dont worry folks i am going to see a therapist tomorrow
for all of this. i need to get it under control = )
i was in a differnet support group and then i found this one. the
other one was rather negative. VERY negative to be quite honest.
these young girls would ask questions and these ladies would pretty
much tell them they are diseased and shouldnt have sex anymore and
are at high risk for gettting cancer. no one should have to go
through that type of torment. cancer is the LAST thing one should
worry about. especially when the cells are detected this early, IT
CAN BE TAKEN CARE OF! (thats the one thing im trying to get through
my head! lol) that group seemed to be scaring people more than
really INFORMING people...so i chose to leave. & here i am.
IVE decided to step back and take a look at my life. ive become more
healthy, taking vitamens, and have begun to eat better. my mother,
aunt, and grandmother all had this and all had the cells frozen off
with no reoccurances. im getting that done in 2 weeks.
just so everyone who is as scared as i was out there knows....HPV,
especially dysplasia...is NOT the end of the world. it is treatable,
and life WILL go on. everyone keep Ur head up. trust me, i am a
hypochondriac, and i KNOW how hard it is =)
2 questions for all of you: #1 should oral sex be continued or
stopped? what are the odds of oral cancer? (my doctor didnt mention
a damn thing) and #2 LETS HEAR SOME SUCCESS STORIES! = )

Hello all, I was diagnosed with hpv last year because of my GW. I got
treated with aldara and they went away. Now the warts are back this
year. I am back on aldara but am afraid and wondering if this means I
can't stop shedding or getting infected. I spoke to my boyfriend last
year and he doesn't seem as concerned as I am. He went to his doctor
and got the warts frozen and his went away. When I talk to my gyn,
she doesn't seem as bent out of shape as I am. I keep thinking that
maybe my immune system is bad or something. I am the kind of person
who gets colds easy. What do you do to build a stronger immune system
to fight off this hpv? or am I asking the right questions?? I am so
confused.

Guys well I do appoligize to everyone for giving wrong infromation but I am just stating what happened in my situation. I know exactly who I got it from because it was my first sexual partner, and I know who he got it from because she is the only one he slept besides me. And I believe him 100% because I was with him for 7 years and he admitted to cheating on my once but waited till after we slept together to tell me. But anyways, this girl that we got it from I talked to her after because I was really concerned about it because I heard she has cervical cancer and since I have the same strand as her. Anyways she had low risk hpv (the cancer was not cause by the hpv) and she had an outbreak when she was 13 of warts and never got them again so she thought that she couldn't give it to anyone, but my boyfriend didn't get it just me so this is why I believe that once you have it can always spread it because it is virus that you have for life. Not everyone you sleep with will get
it because 80% of people who have it don't even know it and never will. And those of us who get most of the time only have it a handfull of times if that and our bodies controll it.

Cheers, Ryan, I agree with you 100% and I think more
people are beginning to look at it from that point of
view, given the current knowledge and studies.
There will always be unique circumstances with any
virus and/or disease, but we should not base our
opinions and decisions on those situations alone. To
my knowledge, the CDC and others are the only real
studies that are current. Correct? Therefore it
seems only reasonable that people's opinions and
decisions should be based off of those, more so than
many of the stigmas surrounding hpv (that I am not
sure anyone can point to a study that concludes the
life long threat).

In my opinion you are being waaaaaay to paranoid. I dont think a kid
sitting in your lap will transmitt the disease and i am sure it wont live in
your bedsheets for any amount of time. There must be some direct contact of
the infected person and the other person. Be careful but dont kill your
self worrying so much!!

Hello, I am a male and I have had HPV for a while now, but I'm still
not too clear about some certain issues, can someone help? I am
clear on the fact that HPV can be passed through sexual contact, even
when using a condom because of the skin to skin exposure that
occurs. However, when I go to different sources, I still seem to get
different answers about other ways HPV can be passed.
For example, one source may say that HPV cannot be passed on by
sharing towels with someone, while another source may say it is
possible. I'm tired of being PARANOID! I have been very clean and
make sure that my hands are always clean, etc. But I'm wondering if
there are certain cleaning behaviors on my part that might be
unnecessary. Can simply rolling around naked in bed for example,
with certain body areas being exposed to the sheets, be enough to
pass on to someone else who decides to roll around naked on the
sheets themselves? And what about sharing toilets? If someone has
HPV and their genitals touch the toilet seat, and then someone else
who is not infected uses the toilet also and their genital touch the
seat, can it be passed on that way as well even if visual symptoms
are present?
I realize that when visual symptoms are present the chances of
passing it are higher. So does that mean that when symptoms are
present and all the examples given above are applied, the virus can
still be passed this way? I'm very confused. If that is the case,
then sources should never say that the only way HPV can be passed is
through sexual contact.
On a similiar note, I've heard that the inner thighs are also an area
that symptoms can occur, or where exposure can be passed on. I guess
cause they're near the groin area, I'm not sure. Does this mean that
I need to make sure and wear long shorts so that I don't expose my
upper thigh to anyone who is not infected? How far do I go with
these cautionaries? How far along my upper thighs does it become a
concern? I have kids to worry about too. If I'm wearing short
shorts, or maybe it's morning and I'm still in my underwear, and my
kids are in their underwear, and they come to sit on my lap, are they
in any danger of be exposed to HPV? As you can see I still have many
questions and concerns. Please, if anyone can help, it would be very
much appreciated.

Stimulating the Skins Immunity to Help the Body Repair and Recover From Viruses and Common Skin Conditions

NEW YORK (October 22, 2003) Of all the things the skin does each day protecting the vital organs, muscles and skeleton, and controlling body temperature and fluids one of its most important functions is the ability to defend the body against viruses that can cause illness. In the past, treatment options were not advanced enough to use the skins own immunity to assist in the resolution of both common and life-threatening conditions. However, recent understanding of the immunity of the skin has resulted in the development of treatments that use the bodys immune system to fight illness and resolve common conditions, including many dermatologic skin conditions.

Speaking today at the American Academy of Dermatologys (AAD) Derm Update 2003, dermatologist Brian Berman, MD, PhD, Professor of Dermatology and Internal Medicine, University of Miami School of Medicine, Miami, Fla., discussed the new agents that dermatologists are using to boost the skins own immunity to safely and effectively treat some skin conditions.

The benefit of these new therapies is that they use the bodys own defenses to successfully control and potentially cure some of the most common skin conditions conditions that can be both physically and psychologically disruptive to patients lives, said Dr. Berman. These therapies can be used in two unique ways: to optimize and enhance the skins immunity to resolve viral infections and skin cancers, or to control abnormally excessive immunity reactions that are common in skin conditions, such as psoriasis and atopic dermatitis.

Immune Response Modifiers

One class of these topical agents is Immune Response Modifiers (IRMs), which enhance the skins ability to identify and control, or destroy, infections, bacteria and other foreign objects in the body responsible for illness. IRMs work by stimulating cytokine production in the skin. Cytokines are naturally occurring proteins which are used by immune system cells to communicate with each other. When cytokines are stimulated by IRMs, they enhance cell immunity, a natural process to help the body control or eliminate virus-infected and tumor cells. Because IRMs assist the immune system as a whole rather than target specific viruses or tumors they have potential use in the treatment of a wide variety of dermatologic diseases.

One IRM, imiquimod, was approved by the Food and Drug Administration (FDA) for the treatment of genital warts caused by human papillomavirus (HPV) infections. Although they usually do not hurt or itch, genital warts can be unsightly and embarrassing, and are associated with the development of genital cancers. While current treatments involve destroying the visible warts by freezing, burning, or applying acids, the problem can recur after these treatments because the virus that causes the warts can persist in the skin.

The use of imiquimod prompts the body to fight off the virus that causes genital warts. In fact, a recent study showed that the use of imiquimod, in a 5 percent cream, increased the levels of cytokines in the skin enhancing the antiviral state. Over half of patients who applied imiquimod three times a week for up to 16 weeks completely cleared their genital warts.

Before the approval of imiquimod, the only patients whose genital wart infections were truly cured were those whose immune system recognized and eliminated the virus. However, this was not a common occurrence, stated Dr. Berman. This new treatment can assist many more patients by boosting the bodys immunity to destroy this virus, reduce the need for surgery and destructive ablations.

Another potential use for IRMs is to treat keloids, the excessive scars that result from alteration in the normal wound healing process following skin injury, including surgery. These scars can occur in any individual but are most common in people with darker skin. Keloids form when alterations occur in the normal control of skin fibroblast cell function, which are the cells that make the structural fibers of the connective tissues in the skin. One of the naturally occurring immunomodulators, or anti-inflammatory agents, in the skin is interfreron which can normalize the skins wound-healing function. The use of imiquimod has been shown to induce the production of interfreron to prevent the keloid from returning following surgery.

These disfiguring scars are tender, can cause pain and burning, and can be psychologically challenging for the individuals affected by them, said Dr. Berman. Current treatment involves removing the scars surgically, which more often than not causes the scars to recur, often larger than the original keloid removed. The use of IRMs to prevent keloids offers hope to those individuals who struggle with these scars.

IRMs also show promise for the treatment of skin cancer and actinic keratoses, the precursor to a form of skin cancer. Basal cell carcinoma (BCC) is the most common form of skin cancer, accounting for more than 80 percent of all skin cancers each year. If detected and treated early, BCC has a 95 percent cure rate. However, if left untreated, it can grow into nearby areas and invade the bones making successful treatment more difficult. Current treatment methods for BCC include invasive surgical removal and destruction.

However, a recent study of the IRM imiquimod showed an excellent tumor-clearance rate in patients with BCC. Patients were treated five times a week for six weeks with the IRM before undergoing tumor excision. Following their surgery, eighty-two percent of those who were treated with imiquimod were found to have no additional evidence of the tumor.

Actinic keratoses (AKs) are common small, scaly spots that develop on parts of the body that have been exposed to the sun and that if left untreated, have the potential to progress to squamous cell carcinoma, the second leading cause of skin cancer deaths in the United States. While current therapies to treat AKs include cryosurgery, surgical removal and biopsy, topical chemotherapy, and photodynamic therapy, IRMs have been recently successful in treating AKs. In fact, a recent study showed an 83 percent median clearance rate of AKs when patients applied imiquimod twice a week to the affected area.

The incidence of skin cancer is increasing at an alarming rate in the United States and these new options are showing promise in treating skin cancers and actinic keratoses safely and effectively with minimum downtime for patients and excellent cosmetic outcomes, said Dr. Berman.

Topical Immunomodulators

There are many diseases that result from abnormally excessive immune responses, such as atopic dermatitis or eczema. For over a decade, potent systemic immunomodulators have been used for the treatment of eczema, a chronic skin condition characterized by itchy, inflamed skin typically on the insides of the elbows, backs of the knees and the face. But in the past two years, these medications have been incorporated into topical preparations called topical immunomodulators (TIMs) that exert their powerful anti-inflammatory effects on the activation of T-cells, a type of white blood cell in the body responsible for triggering immune responses. These treatments also target the skin without hampering the immune systems ability to defend itself from bacteria, viruses, and other diseases. Two TIMs, tacrolimus ointment and pimecrolimus cream, have been approved by the FDA for the treatment of atopic eczema.

TIMs have been shown to effectively treat eczema and common skin disorders, stated Dr. Berman. Because they are steroid-free they do not have the traditional side effects such as thinning of the skin, formation of dilated blood vessels, stretch marks, and infection.

Biologics

Treatment of psoriasis, a recurring, non-contagious skin disorder that is characterized by raised, thickened patches of red skin covered with silvery-white scales, has recently benefited from the development of biologic agents. Biologic therapies block T-cell activation, which in psoriasis release cytokines to tell skin cells to reproduce and proliferate at an accelerated rate, thereby setting off other reactions that lead to psoriatic lesions forming on the skin.

The first biologic to be approved by the FDA for the treatment of psoriasis is alefacept which works by destroying activated T-cells, thereby stopping the early cycle of psoriasis. In a recent study, after patients received a 12-week course of alefacept, whic
